Not only are we responsible to educate your child, but we also desire to keep your
child safe while he/she is under our supervision. Therefore, we kindly ask that
you download these forms, complete this paperwork, and return it to us by Friday,
August 1, 2008. Please drop it off at the Lower School office or mail it to Whitefield

Georgia law requires that each student has a “Certificate of Immunization Form 3231”
on file in order to be enrolled in school. This form must be obtained from your
child’s doctor’s office, your child’s former school file, or the local Health Department.
This form must be on file in order for your child to attend school.
