These forms must be on file by August 1, 2009. Students will not be able to attend the first day of school without all documentation.
Forms should be returned to the Administrative Office at Whitefield Academy, 1 Whitefield Drive SE, Mableton, GA 30126.
Please write “Required Forms” on the envelope.
If the forms are not completed correctly, they will be returned for you or the physician to fill out properly.
Please note that each student's Certificate of Immunization (Form #3231) must be updated and on file each year. This form
can be obtained from the student's doctor's office, former school file, or the local health department.
Physicals completed before April 1st are valid for one year from date. Physicals completed on or after April 1st are valid
for the entire following school year.
Parents can check when their child's physical expires under the
parent login.
