Health Services
All medical forms should be completed through the Magnus Health Care website. For questions or support with the website, please contact Magnus at 877-461-6831.
Please complete the following forms online:
- Vital Records – This information will remain active as long as your child is enrolled at Whitefield. You will be responsible for updating personal or medical information throughout the school years as any changes may occur.
- Permission to Contact Physician, Release for Emergency Treatment, participant Consent, Release and Wavier
- Concussion Awareness
- Cardiac Arrest Awareness
- Over the counter medications
The following forms cannot be completed online. You must print the Annual Physical form, have your child’s physician complete and sign, then fax, or scan the form directly to Magnus Health Care with the assigned Magnus facesheet, one per form. These forms are good for one year from the date of your child’s annual physical.
- Annual Physician form (must be signed by physician)
- Georgia Immunization form #3231 (form provided by your physician)
After completing the online Vital Records questionnaire, you may be directed to print and sign one or more of the following action plans depending upon your child’s health history. These forms must be printed and updated by your child’s physician during your child’s annual physical. Fax or digitally scan and upload form (s) to Magnus Health Care.
- Authorization to Give Prescription Medication
- Authorization to Carry Emergency Medication (Grade 5-12)
- Asthma action plan
- Food Allergy action plan
- Diabetes action plan
- Seizure action plan
- Bee sting action plan
